Remove the lug nuts and wheels.
Take out the brake caliper assembly.
1. Remove the brake rotors.
2. Install the new rotors.
3. Grease the caliper and reinstall it.
4. Bleed the braking system, to get any residual air out of the brake lines.
5. Reinstall the wheels and tighten each lug nut until secure.
